Paediatric OT and case management

Journify runs children's OT and adult case management from the same system. Sensory histories, COPM scores and sensory diets sit alongside school and LA funding, and itemised time is billed monthly to case managers and solicitors.

Regulated by the HCPC (protected title: occupational therapist). Professional body: RCOT, plus BABICM for brain injury and complex case managers.

What it does for occupational therapists

Time billed in 6 minute units

Case management mostly happens outside appointments. It's calls, emails, letters, travel and meetings, billed monthly in arrears to the case manager or solicitor. Journify logs that time in 0.1 hour units with an activity code, no diary slot needed, and pulls it straight into the monthly report and the invoice.

  • Activity codes for call, email, letter, travel, meeting and report
  • A rehabilitation budget for each case, tracked against what's been spent
  • Billing that can wait until settlement when the case needs it

INAs and home visits, room by room

The Immediate Needs Assessment template follows the order you'd write it in: injury and background, function by domain, home, equipment, care, risks, then recommendations with costs. The home visit form has fields for door widths, step heights, and bed, chair, bath and toilet heights, plus a risk log. Case files can end up disclosed in litigation, so every entry is dated and signed, and corrections keep the original visible.

  • An equipment schedule: recommended, approved, ordered, delivered, reviewed
  • Equipment justification letters built from the schedule
  • A lone working risk assessment for home visits

Sensory diets families can follow

The sensory diet builder schedules activities by time of day, with a purpose and a duration for each, so home and school get something they can actually use. Handwriting samples go in as dated photos, so you can put September's next to March's. The parent sensory history and the teacher questionnaire go out to several people at once, before you've even met the child.

  • COPM with change scores worked out for you
  • Goal Attainment Scaling as the default goal tool
  • EHCNA advice laid out as Sections B, E and F, with a deadline clock

Who pays, and how it's billed

Parents, schools and local authorities pay for most children's work, while adult work is funded by case management companies, solicitors or defendant insurers, usually monthly in arrears. Treatment by an HCPC-registered OT is VAT exempt, but reports that quantify the costs of disability for a compensation claim, and expert witness work, are standard-rated.

Questions from occupational therapists

A case has a deputy, a case manager, two solicitors and an insurer. Who sees what?

You record sharing consent for each recipient, and Journify checks it before a report goes out. If there's no consent on file for that person, it stops and tells you, rather than sending.

How does it handle VAT on expert reports?

Expert witness and medico-legal reports are their own services, set to standard-rated by default, so they don't get mixed in with exempt treatment. You can override a line if you need to, but you'll be asked for a reason and it's kept on the audit trail.

I only see children. Do I need the case management side?

No. The paediatric setup is the default. The INA, home visit and monthly report templates come with the adult and case management variant, which you can switch on later if your work changes.

What does it cost?

Prices are still being finalised, so they're TBC for now. Practice is priced per practitioner, with cheaper part-time seats for associates doing up to 40 appointments a month. Admin and reception users are free: one on Solo, as many as you need on Practice and Clinic.
